Community Health Survey
We plan to conduct a Community Health Survey to gain a deeper understanding of the health concerns, priorities, and challenges facing residents in our region. This effort aims to go beyond assumptions by hearing directly from community members about issues that matter most—whether it’s access to care, chronic disease, mental health, or environmental health. The results will help identify key health disparities and guide the development of targeted, community-informed public health programs and initiatives.
Chronic Disease Hotspot Mapping
We plan to use geographic data from the Community Health Survey to identify areas in our region where chronic diseases—like asthma, diabetes, and heart disease—are most concentrated. By mapping these “hotspots,” we can begin to understand the underlying factors contributing to health disparities, such as access to care, environmental exposures, or social and economic challenges. This project will help us design targeted public health programs that bring resources and interventions to the neighborhoods that need them most.
Regional Infectious Disease Dashboard
We plan to develop an interactive dashboard to monitor reportable infectious diseases across our region in real time. You will be able to see data from the region as a whole as well as town-specific data. This tool will help public health staff, boards of health, providers, and residents stay informed about circulating illnesses like flu, COVID-19, and foodborne infections. By turning surveillance data into accessible graphs we can identify patterns quickly and respond with timely interventions. The dashboard will be regularly updated and publicly available on our website, supporting transparency, faster response, and data-driven decisions to protect community health.
